Elizabeth Krzyzanowski, Dental Coordinator

Elizabeth is certified in Oral and Maxillofacial Surgery Anesthesia and X-Ray, and earned her Associates Degree in Medical Assisting from the University of Montana in 2006. She has training in chronic pain management and more than four years of experience in the dental field. Elizabeth is currently the Dental Coordinator for our dental facility, responsible for coordination of care for two full-time dentists.
Executive Director - Cindy Smith, RN, BSN

Mrs. Smith has more than fourteen years of management experience working with not-for-profit healthcare systems. She has extensive knowledge of nursing, public health, emergency preparedness, business planning, operations management and management control systems, contract administration, health economics, and financial reporting and analysis.
Mrs. Smith s experience includes three years of service as the Community Health Center s Executive Director, seven years with Hill County Health Department as the Executive Director, and four years as the Charge Nurse in the Transitional Care Unit for Northern Montana Hospital.

Mrs. Smith earned her Bachelor's Degree in Healthcare Administration in August of 2009, attended the UCLA/Johnson & Johnson Healthcare Executive Program in July of 2009, and earned her Associate's Degree in Nursing in May of 1996. Mrs. Smith is also a licensed Registered Nurse in Montana.

In addition to Mrs. Smith's extensive experience, she is also an active system partner with the following organizations:

Community Health Association of Mountain/Plain States (CHAMPS)

Carol Milne s background includes 28+ years as a Registered Nurse and 9 years as a Family Nurse Practitioner.  She completed FPN degree at the University of Oregon in 2002 and worked in various rural clinics in Oregon.  In 2008 she made the decision to move back to her home state of Montana and took a position in Cut Bank, MT.   Carol joined the Bullhook team in June 2011.

Arlys Williams  received her Pediatric Nurse Practitioner s education and a MS in Nursing from the University of Colorado. She has worked as an Assistant Professor of Nursing for MSU Northern since 2005.  She has been a part of our team since 2009 and continues to teach as well.

Terri graduated from MSU Northern in 1995 with an Associate s Degree in Nursing.  She then went to work for Northern Montana Care Center and worked there until joining our team in 2010. 
Melissa Young, D.D.S.

Dr. Young graduated from Rose-Hulman Institute of Technology with degrees in Chemistry and Chemical Engineering in 2001.  She spent several years working in Research and Development at Eli Lilly & Co. before deciding that patient care is what she wanted to do.  Dr. Young graduated from Indiana University School of Dentistry with a Doctorate in Dental Surgery and completed a General Practice Residency at the University of Toledo.  The residency focused on pediatrics, oral surgery and prosthodontics.  Dr. Young is new to Montana having lived in many other places as part of a military family.

Debby Knudson-DiPiero is a native to Havre and has worked with the Havre community for over 24 years. She received her degree in Addiction Studies in 2006. Deb has a background in Law Enforcement and has worked with the Domestic Abuse Task Force. She has been counseling clients with addiction for over 6 years. Deb is a member of NAADAC, the Association for Addiction Professionals. Deb joined the Bullhook team February of 2012 and has worked with the state of Montana to have Bullhook Community Health Center to be a State Approved facility to provide the ACT Program.

Rachel Thompson, Registered Dental Hygienist

Rachel was raised in Harlem and Chinook, and attended Dental Hygiene school at Montana State University COT-Great Falls to become a Registered Dental Hygienist, she graduated in 2010. She was fortunate to partake in various externships where she was able to treat and educate patients through oral hygiene care at various locations such as Sletten Cancer Center in Great Falls, Fort Harrison Veteran Affairs in Helena, and the Nexus Methamphetamine Treatment Center in Lewistown. She took her first position as a Registered Dental Hygienist (RDH) with Bullhook Community Health Center in 2010 and thoroughly enjoys treating patients of all ages and engaging in community outreach to educate on oral health.

Dr. Donna Smith, DO

Dr. Donna Smith is a family practitioner joining Bullhook Community Health Center from Libby, Montana.  She has been serving  Montana communities since 2000. She is a graduate of Arizona College of Osteopathic Medicine and completed her residency at Rapid City Regional Family Medicine Residency.   During her residency, Dr. Smith  trained at Primary Children’s Hospital in St. Lake City, UT,  Minneapolis Children’s Hospital, and City Heart Hospital in Dehra Dunn, India.  She is Board Certified in Family Medicine.  Her area of focus is childhood and adolescent obesity prevention and patient education.
