Payment of services
Bullhook Community Health Center accepts all private medical insurance, Montana State Medicaid, Medicare, and private pay patients. All patients are required to pay a $15.00 nominal fee at the time of service unless your private insurance, Montana State Medicaid, or Medicare co-pay is less.
Patients that qualify financially based on their gross household annual income and family size using the federal guidelines for poverty will be granted a discounted fee for service. To see if you qualify please see our sliding fee scale qualifications.
