We aim to be your medical home model, providing care from pre-conception (before birth) through the geriatric (elder) phase of life, with age and gender-specific care available, such as care specific to women's health (annual PAP exams and breast exams), and children's health (annual well-child check-ups and vaccinations).

We combine timely preventative care, education, counseling, case management, and urgent and primary care to ensure that our patients and their families have the information and tools necessary to take control of their heath and increase the quality of life on the Hi-Line. We currently provide referrals for patients in need of significant behavioral healthcare, such as those patients with chronic pain or significant depression.
